Tadalafil has an interesting cardiovascular research signal, but the studies reviewed here do not establish it as a longevity treatment for healthy men with normal erectile function. The key uncertainty is not whether it affects a blood-flow pathway. It is whether prescribing it to that different population improves outcomes enough to outweigh harms.
Tadalafil inhibits phosphodiesterase type 5, influencing the nitric-oxide/cGMP pathway. The Cialis prescribing information covers erectile dysfunction, benign prostatic hyperplasia and their combination—not general anti-aging. Normal erections do not rule out a separate clinical indication such as BPH, but that is different from taking it solely for longevity.

The study behind the dramatic longevity headline
The Kloner et al., 2024 tadalafil outcomes study was a retrospective analysis of men with erectile dysfunction. Tadalafil exposure was associated with lower major cardiovascular-event risk (hazard ratio 0.81) and all-cause mortality (hazard ratio 0.56). It was not a randomized prevention trial in healthy men without ED.
A hazard ratio of 0.56 is often repackaged as a 44% longevity benefit. That wording loses the study design, baseline risk and population. Our interpretation is narrower: the result justifies better prospective research, but cannot supply an individual's absolute benefit, number needed to treat or added years of life. The study disclosed industry funding and author relationships; those belong in the interpretation, not hidden behind the headline.
Interpretation limit: Converting an association into a promise that starting the medicine will reduce your chance of death by the same percentage.

Why men without ED are a different question
The 2024 study of PDE5 inhibitors, mortality and cardiovascular outcomes also examined clinical populations involving erectile dysfunction or lower urinary tract symptoms. These diagnoses and the way treatment is selected can differ from a healthy person seeking an anti-aging intervention.
Men who receive and continue a prescription may differ in healthcare access, activity, underlying illness, contraindications and adherence to other treatment. Statistical matching addresses recorded characteristics, not every unmeasured difference. We therefore do not transplant the cohort result into a recommendation for someone whose only goal is optimization. A useful research question would explicitly enroll that population and measure clinical outcomes, adverse effects and discontinuation prospectively.
Interpretation limit: Assuming a larger database fixes a mismatch between the people studied and the people a marketing claim targets.

Safety findings are not proof of preventive benefit
The pooled tadalafil clinical-trial safety analysis examined cardiovascular safety across tadalafil trials. The pooled antihypertensive-safety analysis examined hypotension and cardiovascular events with concomitant antihypertensive medication in studied ED/BPH settings. Such analyses address important safety questions but do not demonstrate longer life in otherwise healthy users.
Imagine a treatment with a low adverse-event rate but no demonstrated benefit for the goal being pursued. That can still be a poor tradeoff. Conversely, a medicine may be reasonable for an established indication despite uncertainty about unrelated prevention claims. Keeping benefit evidence and harm evidence in separate columns makes the decision more honest than declaring the medication simply good or bad for longevity.
Interpretation limit: Treating tolerated in studied patients as equivalent to beneficial for everyone.

The safety facts a blood-flow pitch must not omit
The Cialis prescribing information contraindicates use with nitrates and guanylate-cyclase stimulators such as riociguat. Blood-pressure effects and interactions, including with some antihypertensives and alpha-blockers, require review. Headache, indigestion and back pain are among recognized adverse effects.
Before any prescribed use, the clinician needs the actual medication list and relevant medical history. This review is not a complete contraindication checklist. An erection lasting more than four hours or sudden vision or hearing loss requires urgent medical attention under the product guidance. Do not stop necessary cardiovascular medicines or combine erectile-dysfunction medicines based on a longevity article.

For the present decision, write two separate questions: is there a diagnosed problem for which tadalafil is appropriate, and is there independent evidence of the additional benefit being advertised? What Bryan Johnson's current protocol establishes
The Blueprint protocol page, checked September 18, 2026, lists tadalafil among Bryan Johnson's prescription medicines. That supports a carefully limited statement about what he publicly reports using. The same page describes many concurrent interventions, so it cannot isolate tadalafil's contribution to any reported biomarker or outcome. It is a changing personal protocol, not a controlled prevention study.
His Immortals tadalafil page also promotes the medicine and cites cardiovascular studies. This is relevant because it explains why the question is reaching a wider optimization audience. It is also a commercial source with an interest in enrollment. We link it as the claim being evaluated, not as independent evidence of longer life or an endorsement of the service.
Our approach is neither copy the founder's stack nor dismiss a hypothesis because a business sells it. Ask which part of the claim is supported by a label, which by a study, and which by personal experience. A highly measured individual still does not provide a counterfactual showing what would have happened without the intervention. We therefore do not reproduce his medication amounts as instructions for readers.
Trace the mortality headline back to its actual denominator
The 2024 Kloner study behind the prominent claim compared 8,156 tadalafil-exposed men with 21,012 unexposed men who had ED. Its reported mortality association was a hazard ratio of 0.56, not an experiment randomly assigning healthy men to preventive tadalafil. Read the study's exposure definitions, follow-up and disclosed commercial relationships before treating the headline as a personal forecast.
A relative hazard is not an extra number of years of life. Nor can an absolute benefit be calculated just by applying the headline percentage to a reader's age. That would require a valid baseline risk, an appropriate population and evidence that the difference is causal. The distinction is the central value of this review: an interesting association can motivate research without supplying a prescription rationale for every interested person.
A gym pump and improved aerobic performance are different endpoints
Derek's 2019 More Plates More Dates discussion of vascularity describes a bodybuilding perspective and personal preference for Cialis in that context. It is a useful firsthand account of what the community notices and values. It is not a clinical study, and the page also has commercial supplement affiliations. We do not reproduce its drug/supplement combinations or assume that a dated discussion proves uninterrupted years of use.
For counterevidence, a double-blind crossover study in 14 healthy athletes found that acute tadalafil did not improve maximal oxygen uptake or aerobic and anaerobic thresholds under normal-oxygen conditions. This small, short-term experiment is not a chronic low-dose longevity trial either. It simply prevents the leap from a plausible blood-flow mechanism or visible pump to a claim of demonstrated aerobic-performance benefit.
The broader lesson is to define the endpoint before arguing about whether the drug works. A pump, erectile function, exercise tolerance, endothelial measurements and mortality are not synonyms. A favorable result for one can coexist with a null result or no adequate evidence for another. That is not a contradiction to explain away; it is a more accurate map of the research.
Clinical perspectives are most helpful when they identify the indication
Kyle Gillett's hormone discussion on Huberman Lab (2022) discusses male hormone and sexual-health questions in a clinical context. The related hormones and vitality episode discusses tadalafil in relation to prostate and urinary concerns. We treat these as explanatory conversations, not as professional advice to the reader or proof of general anti-aging efficacy.
This distinction gives a useful answer to the original question about solid erections: normal erectile function does not settle whether there is some other diagnosed problem to treat, but it also does not create a preventive indication. The clinical discussion and the bodybuilding discussion are answering different questions. Listening to both is useful only if the article keeps those questions separate and places labels and outcome studies above anecdotes in the evidence hierarchy.
How to follow community claims without turning them into a stack
For a claim encountered on a podcast or X, save the original link and date, the precise outcome being claimed, and the cited paper.
