The question and the answer

You do not need to complete eight worksheets before a conversation can be useful. Start with the page that captures your most important unanswered question.

The PeakedLabs decision workbook is a printable companion to the September 5, 2026 research edition of The Complete Guide to Peptide Therapy. Its pages organize notes for discussion. They do not diagnose a condition, select a treatment or provide a validated safety score.

The Complete Guide to Peptide Therapy

Bring a clearer set of questions to your clinician conversation. The 69-page research guide explains its evidence, provider-check and cost framework; the separate eight-page printable workbook gives you space to record your goal, unresolved questions, written costs and agreed follow-up. $29 USD once for both PDFs.

September 5, 2026 research edition.

Educational, AI-assisted research; no independent licensed clinician review. No dosing, mixing or injection instructions.

Begin with a question, not a finished conclusion

Worksheet 1, “My decision brief,” asks you to describe the problem you want help with without starting from a drug name.

It also has space for what has already been evaluated or tried, practical constraints and the question that matters most to you. You can leave a field unanswered. The purpose is to make the uncertainty visible, not to manufacture a complete record before the appointment.

Interpretation limit: A filled-in page is a discussion aid, not a diagnosis or a treatment decision.

Questions to bring to your clinician

  • What is my most important unanswered question?
  • Which practical constraints should I explain?

Keep the evidence and its limits together

Worksheet 2 records the proposed product, formulation and route, then asks who was studied, what was compared and which outcomes were reported.

It separates “This evidence supports” from “It does not establish.” If you cannot answer a field from the source you have, mark it unresolved and bring the question to the conversation.

Interpretation limit: Completing a worksheet does not establish that an offer is suitable for you.

Questions to bring to your clinician

  • What does this source actually support?
  • Which part of the proposed offer does it leave unanswered?

Give unknown costs a place on the page

Worksheet 4 separates introductory and ongoing charges, required and optional items, and costs included elsewhere.

It has an explicit space for unknown costs and who will confirm them. An unanswered charge is not zero. For a worked fictional example, read the free three-page guide sample: the cover, contents and one cost comparison.

Interpretation limit: The sample figures are teaching examples, not current clinic quotes or promised savings.

Questions to bring to your clinician

  • Which charges are included elsewhere?
  • Who can confirm each unresolved cost?

Leave the appointment with responsibilities recorded

Worksheet 6 includes fields for the agreed clinical question, the outcome to discuss, any agreed observations or tests, who reviews them and when.

Fill these from the conversation with the clinician rather than treating the blank form as instructions to order tests or change treatment. Worksheet 8 returns to the original goal at follow-up or renewal, with space for what changed, other possible explanations, practical burden, actual spending and the next agreed action.

Interpretation limit: The form supports a discussion; it does not make the decision for you.

Questions to bring to your clinician

  • Who reviews the agreed follow-up, and when?
  • What remains unclear about the next agreed action?

Put the research to use

The $29 USD one-time bundle contains the 69-page research ebook and the separate eight-page printable workbook. The ebook provides the explanatory framework; the workbook provides places to record your questions and the answers you obtain. The public sample is available without signup. This is educational, AI-assisted research; independent licensed medical and pharmacy review has not been completed. It includes no individualized medical advice, dosing, mixing or injection instructions. Research and product status can change; the edition date does not promise ongoing updates.
